The traditional engine mounting crutch would have to be changed -- the way it's done now it's a really light and effective way to transfer engine forces to the fuselage. It may be a challenge to get something that's as good without going up much in weight.
And the cowling will be different.
As an alternative, you could make a header that comes off the engine and wraps around to the back, to use a 'normal' inverted engine mount and a 'normal' fuse-bottom pipe with a side exhaust. Of course, you'd have to find the header.
