Seems the push rod to the flaps broke in the middle of the threaded part of the push rod. Right at the bell crank. Looking at the break, it was obvious it had a previous crack, there was corrosion and only half was new shiny break.

I always salvage all the parts from broken planes. Then I don't use them.
