No it was storage mode and no the balance plug was not used.
Should I have used it?

The balance plug tells the charger what the individual cell voltage is.
Not using it is probably why the battery is dead.
Having said that, the battery gurus may not like what I have to say next.
I have recovered completely dead lipos by charging for a short time on NIMH mode watching the battery closely and minding the temperature, if it gets hot shut it down.
With the balance plug plugged in you are trying to reach a minimal voltage per cell, I use 3.6 volts per cell.
Then switching to balance mode and fully charge at a nice slow 1-2 amps.
I have successfully revived several batteries this way.
