Stunt Judging Forms - anybody have a copy?

I am updating our files for the Stunt judging forms and need the current set of judging forms. The AMA rules has a single judging form but looking for a PDF copy of the rules with two score sheets on one page. Using the AMA rules I have started the process to put together the Beginner and intermediate/advanced/expert level judging forms with two score sheets on one page.

Looking for the following judging forms, if anyone has a document already set up with 2 score sheets on one page that would be appreciated
